Complete the application below and submit payment to receive your 2024 Hauler License window decals. This license will expire on January 31 every calendar year. 
In order to complete your application, a $35 payment is required. Per Larimer County Licensing Ordinance No. 1991-1
Each business vehicle operated in the process of moving waste is insured to do business in Colorado. I attest I have policy coverage of:
$150,000.00 for bodily injury or death,
$150,000.00 for property damages and
$400,000.00 for injury or the death of persons and property of others.
Each person under my employment:
has the legal right to work in the United States,
is covered by Workman’s Compensation insurance and
has a valid Driver’s License or CDL.
Every vehicle operated is maintained to a regular set of safety standards:
Regular pickups and flatbed vehicles are only used to haul dry wastes
Liquid containing wastes will only be hauled by leak proof vehicles designed for this waste.
All compaction vehicles are maintained to ensure that compaction blades move freely, hopper plugs, and seals are in place, in good condition, and are leak proof.
All open-bed or open-top vehicles will be tarped to cover entire load, during transport to avoid littering on roads.
All sideboards and tailgates are constructed of permanent materials.

 
I certify that I have the legal authority to sign on behalf of the Waste Hauler and that the statements and information contained in this submittal are true, accurate, and complete.
 
I am aware that failure to provide true and accurate and complete information constitutes a violation of requirements of Ordinance No. 1991-1 and is a Class 2 petty offense as provided for in Section 30-15-402(1) C.R.S., and is punishable by a fine of not more than one thousand dollars ($1000.00) for each separate violation and may also result in the suspension or revocation of my license.
